CMS offices are located at 1114 Chambers Road near The Ohio State University in Columbus 1114 Chambers Rd, Columbus, OH 43212 (one block south of the Kenny/Kinnear intersection and one-half block west on Chambers Road – building is on the north side of street).Our customer service hours, for walk-ins, are 10:00 am to 3:00 pm Monday through Thursday, except holidays.
