Customer Shipping AddressIMPORTANT NOTICE: When ordering products from CMS, customers MUST provide a physical street address. All packages are shipped via United Parcel Service (UPS) "ground" within the United States and UPS will not deliver to a P.O. box. Failure to provide a physical street address for shipping may delay the ordering process by as much as three weeks. Purchase OrdersFor all organizational, school, or public agency orders not submitting payment (charge card information or check) with order, please make certain to include a valid purchase order number. Charge Card OrdersFor payment we accept VISA, MasterCard, and Discover. Charges must clear in advance of shipments. Telephone or Verbal OrdersWhen placing a telephone (614.292.4848) or verbal order, in addition to knowing the product information, please be prepared to provide charge card or purchase order information. If purchasing organization permits placing orders without valid purchase orders, after submitting order follow it with a purchase order marked CONFIRMING ORDER. This will prevent order duplication. Orders cannot be shipped until a valid confirmed purchase order is provided. FAX OrdersFax orders can be placed 24 hours a day, 365 days a year by dialing 800.292.4919 and submitting the completed order form. Pickup OrdersCustomers placing orders with the intention of picking up the complete order at the CMS location must notify CMS by phone at least five business days prior to desired pickup date. Bookstores, Firms, or ResellersOrder on an actual need basis. As a not-for-profit, cost-recovery unit the CMS product pricing structure cannot absorb the costs of processing and handling returned items. Youth and Educational OrganizationsOrganizations not having official purchase orders must make payment with charge card or check when submitting order. Sale ConditionsTaxesAll Ohio customers must add 6.75% sales tax to purchase total. At present, all non-Ohio customers are not subject to Ohio sales tax for online purchases. TAX EXEMPTION PROCEDURE: As this eCommerce site does not currently support tax exempt purchases, all Ohio tax exempt customers (including schools) must place their orders via fax (800.292.4919), telephone (614.292.4848), or mail in order to receive tax exemption. Each order must be accompanied with tax exemption certification and payment: charge card number, check, or school or public agency purchase order. Shipping and Handling ChargesBecause the CMS product pricing structure reflects only our development, production, and purchase costs, extra charges cannot be absorbed. Product ReturnsDue to the narrow margin of returns between costs and selling prices of products, the service does not accept returned items, unless an error occurs in filling the order or items are defective. The service will make reasonable adjustments for improperly filled orders or damaged/defective items. Requests for adjustments must be made within 30 days of order shipment.
